Resources for Advance Care Planning Workshop

BEFORE the workshop

Below are the links to the documents and resources that will be discussed in this workshop. (At in-person workshops, these documents would be handed out.) There is no need to read any of this prior to the workshop; however, you may find it helpful to download the documents, and either save them to your computer or print them so that you can refer to them.
  1. Dying With Dignity Canada Advance Care Planning kit (32 pages)
  2. Dying With Dignity Advance Directive form for Manitoba (6 pages – please print double-sided; the first 2 pages are the instructions, so only pages 3-6 really need to be printed)
  3. Manitoba government Health Care Directive
  4. Manitoba Institute for Patient Safety "It’s Safe to Ask" Medication card
  5. Manitoba Institute for Patient Safety Patient Advocate Agreement
  6. Wallet card that you can print, fill out, and laminate
  7. Patient Rights guide (26 pages)
